To make a nomination you need to fill in all the information on this form and then click the ‘submit’ button.
The form has a drop down box for you to select in which award category you wish to make a nomination.
Please choose the award you are interested in and then fill in all the sections of the form.
A few days after you click submit you will be emailed a form to complete. This form will have space for you to provide the information that shows how the nominee meets the criteria for the award.
